The form is the Duplicate Title Application, printed TTL117_0822 in its footer and filed under M.G.L. c.90D § 14. Mail it with $25 payable to MassDOT to P.O. Box 55885, Boston. The signature block is a perjury declaration, not a notarization. A paid-off loan still on record forces the lender to issue an original letterhead release naming every titled owner; signing the application will not substitute. Worcester green-tags, tows at 72 hours, holds the car 30 days and fines the last owner $500, or $700 the second time, at a hearing; clearing it takes a certified bill of sale plus both sides of the title showing the new owner’s name, address and signature. Springfield‘s § 327-14 allows 12 days on your own lot, at $10 to $50 per day after.
